The First World War Memorial in St. Mary's Church, Horncastle, is an oak board consisting of five panels. The centre panel has a carving of the crucifixion. The panels on either side of the central one each contain two columns of names in incised and gilded lettering. These panels commemorate 114 names of those who lost their lives in the First World War.

The Second World War Memorial is placed beneath the above memorial and is of the same material and style although it is made of one rectangular panel positioned horizontally with names in three columns. 22 names are commemorated as having lost their lives in the Second World War.

The names of all those commemorated, with brief biographical details, are provided on the Roll of Honour website.

2 soldiers who served in the Yorkshire Regiment are commemorated on the memorial in St. Mary's Church.

In addition to the memorial in St. Mary's Church, there is a memorial in the old War Memorial Hospital, now a community resource. The same names as those found in St. Mary's Church are commemorated on a plaque inside the entrance to this building, which is located where Linden Road eneters North Street. The names, with brief biographical details, are also provided on the Roll of Honour website.
