The War Memorial for Stockton-on-Tees is a monument built of Portland stone, resting on a concrete plinth. The Memorial is beside the parish church of St. Thomas, and adjoins the town's High Street. The Memorial is a significant feature of the High Street.

A bronze panel bears the words;-

TO THE LASTING MEMORY OF THE MEN OF STOCKTON-ON-TEES
WHO FELL IN THE GREAT WAR, 1914 - 1919.

THEY DIED THE NOBLEST DEATH
A MAN MAY DIE, FIGHTING FOR
GOD, AND RIGHT, AND LIBERTY,
AND SUCH A DEATH IS IMMORTALITY.

SEE YE TO IT THAT THESE
SHALL NOT HAVE DIED IN VAIN.

Beneath this is a bronze panel INSCRIBED;-

THE NAMES OF THE FALLEN ARE INSCRIBED IN A BOOK
OF REMEMBRANCE IN A BOOK LAID UP IN THE ADJOINING CHURCH.

A transcription of the Book of Remembrance made by H M Price in 1923 was copied by P J Bilbrough in 1981. A copy of this is available in the town's Reference Library, and the names are recorded below.
Curiously, the transcription states that there are 1233 entries. However, a total of 1245 entries were found and are listed below.
It should also be noted that in the "Description of the Memorial", a document compiled for the unveiling of the Memorial in , it is stated that there are;-

1245 names, of which
287 are for men of the Durham Light Infantry, and
171 are for men of the Yorkshire Regiment.
